Physical Therapist – Full-Time and Per Diem

Greenwood House Senior Healthcare in Ewing, NJ is hiring for FT and Per Diem Physical Therapist to provide rehabilitation services in the skilled nursing facility and assisted living community. The Physical Therapist evaluates and treats sub-acute and long-term care residents under their provision. This position also requires good organization, time management, verbal and written communication skills, knowledge of Net-Health documentation software (formerly Rehab Optima), experience with PDPM, billing and coding guidelines, and an ability to educate interdisciplinary team members, caregivers, and family members with the skills incorporated during therapy. The Physical Therapist position comes with a competitive pay rate. Key responsibilities include:

  • Diagnose, treat, and document patients per Physician’s orders
  • Maintain consistent and timely communication with residents and/or family/support system while addressing patient concerns and goals
  • Coordinate with the interdisciplinary team – including scheduling, pain management concerns, medications, transfer status, swallowing precautions and cognitive status, or notification of any medical or behavioral changes noted or observed, and document findings

Assistant Executive Director

Greenwood House Senior Healthcare in Ewing, NJ, is hiring an Assistant Executive Director for a Full-Time position. We will consider nurse leaders that are looking for the next step in their career or a Nursing Home Administrator interested in working for a nonprofit organization. What started as a community nursing home for the aged back in 1939, Greenwood House is now a comprehensive provider of wellness-focused senior lifestyles and health care services that includes skilled nursing care, rehabilitation services, assisted living, homecare services and hospice care. What Will I do in my role as Assistant Executive Director? With approximately 300 employees, the primary purpose of the position is to assist in directing the day-to-day functions of this 5-Star facility in accordance with current federal, state, and local standards, guidelines, and regulations that govern the long-term care facility, and as may be directed by the Executive Director to assure that the highest degree of quality care is maintained. Responsibilities:

  • Assist department directors in the development and use of departmental policies and procedures.
  • Interpret the facility’s policies and procedures to employees, residents, family members, visitors, government agencies, as necessary or instructed.
  • Maintain an adequate liaison with families and residents.
  • Assume the administrative authority, responsibility, and accountability of directing the activities and programs of the facility.
  • Act on behalf of the Executive Director during his/her absence.
  • Represent the facility in dealings with outside agencies, including governmental agencies and third-party payers, or provide an authorized representative of the facility when unable to attend such meetings.
  • Recommend to the Executive Director changes in the overall organization structure.
  • Coordinate the facility’s services and activities with department directors.
  • Review and approve the implementation of new procedures and organizational plans within each department.
  • Participate in facility surveys (inspections) made by authorized government agencies.

Qualifications:

  • Bachelor’s Degree required.
  • Must have a minimum two (2) years’ experience in a supervisory capacity in a hospital or long-term care facility.
  • Must possess a working knowledge of long-term care operational standards set forth in the Federal Register, Requirements of Participation.
  • NJ State Nursing Home Administrator license or licensed as a Registered Nurse required.

    • Diagnose and treat patients who suffer from speech, language, voice, cognitive communication impairments and swallowing disorders
    • Maintain constant communication with residents while addressing their concerns and goals and helping them keep positive attitudes

    • Knowledge of and implementation of PDPM, ICD-10 treatment diagnosis codes, 8-minute rule, Routine Therapy and Restorative programs
    • Participates in Care Plans, family meetings and Managed Care Plan updates

    A successful Occupational Therapist with Greenwood House has the following:

    • A master’s degree or doctorate degree from an accredited university in Occupational Therapy is required
    • NJ State Licensure
    • 2-3 years of experience preferred
