How to Apply

Skilled Nursing, Rehabilitation &
Long-term Care:

Sending a completed application does not automatically place an applicant on the waiting list. The following steps have to be completed prior to placement on the waiting list:

• Click here to download the application. Email your completed application to Trish O’Brien at tobrien@greenwoodhouse.org or fax to her attention at 609-883-9252.
• Complete a family interview with the Executive Director.
• Complete an applicant assessment by Greenwood House Staff.
• Arrange a family meeting with Greenwood House Admissions Committee. 

Assisted Living, Abrams Residence:
