We have built strong relationships with several consulting physician specialists who visit Greenwood House upon request, which eliminates the stress of transportation on impaired residents. Our Homecare Program continues to participate in Capital Healths new clinician-led program that transitions residents from hospital to nursing facility to home. We continue to be Capital Healths preferred nursing facility. Kristy Reed, a nurse practitioner specialist from Capital Health, continues to visit all subacute care residents at least three times per week, which has greatly enhanced continuity of care. To date, this program has been very successful as evidenced by the following:Greenwood House continues to treat acute conditions of sepsis in-house (urinary tract infections and pneumonia, for example) with the use of intravenous antibiotics. In addition, the Greenwood House nursing staff monitors and treats medical crises, such as congestive heart failure (CHF) or chronic obstructive pulmonary disease (COPD), and provides the highest quality of care to our residents.Greenwood House, in conjunction with St. Francis Medical Center, has accepted patients from St. Francis who are in the Bundled Payments for Care Improvement (BPCI) initiative. Additionally, Greenwood House has accepted bundled patients from Capital Health. Again, the ability of Greenwood House to provide high-quality, cost-effective care and create a comprehensive discharge plan has been imperative in reducing costs for these BPCI Medicare patients. Greenwood House continues to partner with the Rothman Orthopaedic Institute.LOCAL REFERRING HOSPITALS6'
